زيت نباتي متجذّر في الطب التقليدي، وأدلته في تساقط الشعر أولية وتنحصر تقريباً في دراسة صغيرة على التساقط الكربي.A plant oil rooted in traditional medicine whose hair-loss evidence is preliminary and confined largely to one small study in telogen effluvium.

زيت الحبة السوداء حاضر بقوة في ثقافتنا وفي بيوت كثير من مراجعيّ، وأُسأل عنه أكثر من أي منتج طبيعي آخر. الدليل العلمي المتاح أولي: دراسة صغيرة على غسول موضعي يحتوي زيت الحبة السوداء في حالات التساقط الكربي أظهرت تحسناً في الكثافة مقارنة بالعلاج الوهمي، إضافة إلى خصائص مضادة للالتهاب والأكسدة موثّقة مخبرياً. لا يوجد دليل معتبر على فائدته في الصلع الوراثي. أعتبره خياراً مساعداً مقبولاً في بعض حالات التساقط الكربي أو تهيّج الفروة، لا علاجاً أساسياً.Black seed oil is deeply embedded in our culture and in many of my patients' homes, and I am asked about it more than any other natural product. The available evidence is preliminary: a small study of a topical lotion containing Nigella sativa oil in telogen effluvium showed improved density versus placebo, alongside well-documented anti-inflammatory and antioxidant properties in the laboratory. There is no meaningful evidence of benefit in pattern hair loss. I regard it as an acceptable adjunct in some cases of telogen effluvium or scalp irritation, not as a primary treatment.

ما هو وكيف يُفترض أن يعملWhat it is and how it might work

زيت الحبة السوداء يُستخرج ببرد أو عصر بذور نبات Nigella sativa، وهو من أقدم ما استُخدم في الطب التقليدي في منطقتنا. مكوّنه الفعّال الأبرز هو الثيموكينون، إضافة إلى أحماض دهنية غير مشبعة مثل حمض اللينوليك.Black seed oil is cold-pressed from Nigella sativa seeds and is among the oldest remedies used in traditional medicine in our region. Its principal active constituent is thymoquinone, alongside unsaturated fatty acids such as linoleic acid.

الآليات المقترحة في سياق الشعر ثلاث: خفض الالتهاب حول البصيلة عبر تثبيط وسائط التهابية، ومقاومة الإجهاد التأكسدي بفضل خصائصه المضادة للأكسدة، وتحسين حاجز فروة الرأس الدهني مما يقلل الحكة والقشرة. لاحظ أن هذه آليات تهدئة وحماية لا تحفيز مباشر للبصيلة، وهذا يفسّر لماذا يبدو منطقياً أكثر في التساقط الكربي والفروة الملتهبة منه في الصلع الوراثي الذي محركه هرموني.Three mechanisms are proposed for hair: reducing perifollicular inflammation by inhibiting inflammatory mediators, countering oxidative stress through its antioxidant properties, and improving the scalp's lipid barrier, which reduces itching and flaking. Note that these are calming and protective mechanisms rather than direct follicular stimulation, which explains why it makes more sense in telogen effluvium and an inflamed scalp than in hormonally driven pattern hair loss.

ماذا تقول الدراساتWhat the studies show

الدراسة السريرية الوحيدة التي أستند إليها في هذا الملف نُشرت عام ٢٠١٣ وكانت دراسة تجريبية صغيرة على عدد محدود من المشاركات المصابات بالتساقط الكربي. استُخدم غسول موضعي يحتوي زيت الحبة السوداء بتركيز منخفض مقابل غسول وهمي لمدة ثلاثة أشهر، ولوحظ تحسّن في كثافة الشعر وسماكته لدى نسبة أعلى في مجموعة العلاج. النتيجة مشجّعة، لكن حجم العينة صغير جداً والمدة قصيرة والتركيبة تحتوي مكوّنات أخرى، ولم تُكرَّر الدراسة على نطاق أوسع.The only clinical study I rely on here was published in 2013: a small pilot in a limited number of women with telogen effluvium. A topical lotion containing a low concentration of Nigella sativa oil was compared with a vehicle lotion over three months, and a higher proportion in the treatment group showed improved hair density and thickness. Encouraging, but the sample was very small, the duration short, the formulation contained other ingredients, and it has not been replicated at scale.

أما الجزء الأكبر من الأدبيات فمخبري: عشرات الدراسات تؤكد خصائص الثيموكينون المضادة للالتهاب والأكسدة والميكروبات، وهي خصائص حقيقية وموثّقة جيداً، لكنها ليست دليلاً على نمو الشعر. هذه نقطة أشدّد عليها كثيراً لأن التسويق يخلط بين الاثنين عمداً.The bulk of the literature is laboratory work: dozens of studies confirm thymoquinone's anti-inflammatory, antioxidant and antimicrobial properties. Those properties are real and well documented, but they are not evidence of hair growth. I stress this point because marketing deliberately conflates the two.

في الصلع الوراثي تحديداً لا توجد تجربة عشوائية منشورة على زيت الحبة السوداء، ولا يظهر في الدلائل الإرشادية ولا في التحليلات الشبكية للعلاجات. لذلك لا أقدّمه أبداً كخيار للصلع الوراثي، ومن يفعل ذلك يتجاوز ما تسنده البيانات.In pattern hair loss specifically there is no published randomized trial of black seed oil, and it appears in neither guidelines nor treatment network meta-analyses. So I never present it as an option for pattern hair loss; doing so goes beyond what the data support.

لمن قد يناسبWho it might suit

أقبله بشكل أساسي عند حالات التساقط الكربي بعد معالجة السبب الأصلي، وعند من لديه فروة جافة أو متهيّجة ويرغب بمستحضر ملطّف. كثير من مراجعيّ يستخدمونه أصلاً قبل زيارتي، وموقفي هنا تنظيم الاستخدام لا منعه.I mainly accept it in telogen effluvium once the underlying cause has been addressed, and for patients with a dry or irritated scalp who want a soothing preparation. Many of my patients are already using it before they see me, and my role here is to organize its use rather than forbid it.

لكني أصارح كل من يظن أنه علاج للصلع الوراثي بأن هذا غير صحيح، وأن التساقط الوراثي له محرك هرموني لا يعالجه زيت مهدّئ مهما كانت جودته.But I tell anyone who believes it treats pattern hair loss plainly that this is not the case: hereditary loss has a hormonal driver that no soothing oil, however good, addresses.

كيف أستخدمه في العيادةHow I use it in clinic

حين أقبله كمساند، أفضّل الصيغة الموضعية المخففة على الفروة لا على الشعر، لأن الهدف هو جلد الفروة لا ساق الشعرة. والزيت النقي ثقيل ولزج ويصعب غسله، ولهذا أنصح بخلطه بزيت حامل خفيف أو استخدام غسول جاهز بتركيز معلن.When I accept it as an adjunct, I prefer a diluted topical applied to the scalp rather than the hair, because the target is scalp skin and not the shaft. The neat oil is heavy, sticky and hard to wash out, so I advise blending it with a light carrier or using a ready-made lotion with a stated concentration.

أما الاستخدام الفموي فأكثر تحفظاً منه: البيانات على الجرعة والسلامة الطويلة غير كافية، وهناك تداخلات محتملة مع أدوية السكري والضغط ومميّعات الدم.I am more reserved about oral use: data on dose and long-term safety are insufficient, and there are potential interactions with diabetes, blood pressure and anticoagulant medications.

  1. التخفيف: خلط الزيت بزيت حامل خفيف بنسبة تقارب النصف، أو استخدام مستحضر جاهز بتركيز منخفض معلن.Dilute: blend with a light carrier oil at roughly one to one, or use a ready-made preparation with a stated low concentration.
  2. اختبار الحساسية: كمية صغيرة على الساعد وملاحظتها ٤٨ ساعة قبل التطبيق على الفروة.Patch test: a small amount on the forearm observed for 48 hours before scalp application.
  3. التطبيق: على فروة الرأس مرتين أسبوعياً مساءً، مع تدليك خفيف وتجنّب ساق الشعرة قدر الإمكان.Apply: to the scalp twice weekly in the evening with gentle massage, avoiding the hair shaft as far as possible.
  4. الغسل: شامبو لطيف بعد ساعة إلى ساعتين أو في صباح اليوم التالي، ولا يُترك أياماً.Wash out: a gentle shampoo after one to two hours or the next morning; it is not left on for days.
  5. الفصل عن الأدوية الموضعية: لا يُخلط مع المينوكسيديل أو غيره في العبوة نفسها.Keep separate: never mix it with minoxidil or other topicals in the same bottle.
  6. المتابعة: إعادة تقييم عند الشهر الثالث، ومراجعة تحاليل السبب في حالات التساقط الكربي.Follow-up: reassess at month three, and review the causative workup in telogen effluvium.

ما الذي يمكن توقعهWhat to expect

في التساقط الكربي، الجزء الأكبر من التحسّن يأتي من معالجة السبب الأصلي مثل نقص الحديد أو خلل الغدة الدرقية أو التعافي بعد الولادة أو الحميات القاسية، لا من الزيت. أوضح ذلك مسبقاً حتى لا يُنسب الفضل للزيت ويُهمل السبب الحقيقي.In telogen effluvium, most of the improvement comes from treating the underlying cause, such as iron deficiency, thyroid dysfunction, postpartum recovery or restrictive dieting, not from the oil. I make that clear in advance so the oil is not credited while the real cause is neglected.

الأمان والآثار الجانبيةSafety and side effects

الاستخدام الموضعي المخفف آمن في معظم الحالات، وأكثر ما يُبلّغ عنه ثقل الزيت وصعوبة غسله وانسداد مسام الفروة عند الإفراط. التحسس التماسي وارد لكنه غير شائع، وقد سُجّلت حالات تفاعل جلدي فقاعي شديد مع الاستخدام الموضعي المركّز، وهذا سبب إصراري على التخفيف واختبار الحساسية.Diluted topical use is safe in most cases; the commonest complaints are the oil's heaviness, difficulty washing it out and follicular occlusion with excessive use. Contact allergy occurs but is uncommon, and severe bullous skin reactions have been reported with concentrated topical application, which is why I insist on dilution and a patch test.

الاستخدام الفموي يحتاج حذراً أكبر: قد يخفض سكر الدم والضغط، وقد يزيد أثر مميّعات الدم، ولذلك يجب إخبار الطبيب به قبل أي عملية جراحية.Oral use warrants more caution: it may lower blood glucose and blood pressure and may potentiate anticoagulants, so it must be disclosed to the treating doctor before any surgery.

موقعه ضمن الخطة العلاجيةIts place in the overall plan

في التساقط الكربي يأتي زيت الحبة السوداء في مرتبة متأخرة بعد الأهم: تحديد المحفّز وعلاجه، وتصحيح نقص الحديد والفيتامين د وضبط الغدة الدرقية، وإضافة المينوكسيديل الموضعي عند الحاجة لتسريع التعافي. الزيت هنا مساند مريح لا أكثر.In telogen effluvium, black seed oil ranks well behind the essentials: identifying and treating the trigger, correcting iron and vitamin D deficiency, stabilizing thyroid function, and adding topical minoxidil where needed to speed recovery. Here the oil is a comfortable adjunct, nothing more.

في الصلع الوراثي موقعه صفر عملياً، ولا أدرجه ضمن الخطة. وحين يخبرني المراجع أنه يستخدمه منذ سنة ولم يرَ نتيجة، أوضح له أن هذا متوقع لأن الآلية أصلاً لا تستهدف سبب حالته، ثم ننتقل معاً إلى علاج يستهدفه فعلاً.In pattern hair loss its place is effectively nil and I do not include it in the plan. When a patient tells me he has used it for a year without result, I explain that this is expected because the mechanism never targeted the cause of his condition, and then we move together to a treatment that does.

هل ينفع زيت الحبة السوداء للصلع الوراثي؟Does black seed oil help pattern hair loss?

لا يوجد دليل سريري يدعم ذلك. محرك الصلع الوراثي هرموني، وزيت الحبة السوداء يعمل كمضاد التهاب ومضاد أكسدة، وهذه آلية لا تستهدف السبب. أستخدمه في التساقط الكربي أو تهيّج الفروة، لا في الصلع الوراثي.No clinical evidence supports that. Pattern hair loss is hormonally driven, while black seed oil acts as an anti-inflammatory and antioxidant, a mechanism that does not address the cause. I use it in telogen effluvium or scalp irritation, not in pattern loss.

هل أشربه أم أضعه على الشعر؟Should I drink it or apply it?

في سياق الشعر أفضّل الموضعي المخفف على الفروة، لأن الدراسة المتاحة كانت على مستحضر موضعي وليس على الجرعة الفموية. الاستخدام الفموي يحمل تداخلات دوائية أكثر، خصوصاً مع أدوية السكري والضغط ومميّعات الدم.For hair I prefer a diluted topical on the scalp, because the available study used a topical preparation rather than an oral dose. Oral use carries more drug interactions, particularly with diabetes, blood pressure and anticoagulant medications.

استخدمته أسبوعين ولم أرَ فرقاً، هل أستمر؟I used it for two weeks with no difference, should I continue?

أسبوعان مدة قصيرة جداً لأي حكم، فدورة الشعرة تحتاج شهوراً. لكن الأهم أن تسأل نفسك عن التشخيص: إن كان تساقطاً وراثياً فالاستمرار لن يغيّر شيئاً، وإن كان كربياً فالأولوية لعلاج السبب لا للزيت.Two weeks is far too short for any judgement; the hair cycle needs months. But the more important question is the diagnosis: if this is pattern loss, continuing will change nothing, and if it is telogen effluvium, the priority is treating the cause rather than the oil.

هل يمكن استخدامه مع المينوكسيديل؟Can I use it with minoxidil?

نعم لكن في أوقات منفصلة وليس في العبوة نفسها. الزيت يشكّل طبقة دهنية قد تعيق امتصاص المينوكسيديل إن وُضع فوقه. أفضّل المينوكسيديل يومياً والزيت مرتين أسبوعياً في ليالٍ مختلفة مع غسل جيد.Yes, but at separate times and never in the same bottle. The oil forms a lipid layer that may impede minoxidil absorption if applied over it. I prefer minoxidil daily and the oil twice weekly on different nights with a proper wash.

هو منتج طبيعي وتراثي، لماذا التحفظ؟It is natural and traditional, why the reservation?

تحفظي ليس على النبتة بل على ادعاءات تتجاوز الدليل. الحبة السوداء لها خصائص مضادة للالتهاب موثّقة، وهذا شيء حقيقي، لكن ذلك لا يجعلها علاجاً لتساقط الشعر. وهي أيضاً ليست بلا مخاطر: هناك تفاعلات جلدية وتداخلات دوائية موثّقة.My reservation is not about the plant but about claims that outrun the evidence. Nigella sativa does have documented anti-inflammatory properties, which is real, but that does not make it a hair-loss treatment. Nor is it risk-free: skin reactions and drug interactions are documented.
